Welcome! Thank you for choosing Inspired Dermatology for your skin care. We are honored to care for you. Below you will find some helpful instructions to best prepare you for your first visit.

Instructions

We ask that you sign up for the Patient Portal and complete your paperwork and consent forms prior to your appointment. This is important to reduce your wait time in the office and ensure we have all the information we need to provide you with excellent care.

If you have not already completed your new patient paperwork online, then please arrive at least 20 minutes prior to your appointment to allow time to complete this paperwork on one of our iPads. If you arrive after your appointment time, you may have to reschedule.

All patients (except for those on Medicare) are required to leave a credit card on file with our practice in case there is an unpaid balance after receiving payment from the insurance company.

Items to bring to your appointment are:

  • Valid Photo ID
  • Insurance Card(s), both Primary and Secondary
  • Valid Credit Card/HSA Card
  • List of Current Medications
  • Name and Location of your Preferred Pharmacy
The insurances accepted by us can be seen here. However, even if we are generally in-network for an insurance company, the company may not allow us to accept all plans, so please confirm that our physician is in-network with your specific insurance plan prior to being seen. This can be done by calling the customer service number on the back of your card or logging into your insurance company’s website and checking through their system.

If your insurance requires a referral, please ensure that we have this on file prior to being seen. Without it, your visit may not be covered and this can result in a large out-of-pocket expense for you.

If we do not participate with your insurance carrier or if you do not have insurance, you have the option to pay a discounted self-pay rate which is due on the date of service. For info on Good Faith Estimates for self-pay patients, please click here.
